We are a very reputable hospice agency seeking a compassionate and experienced Registered Nurse Case Manager to provide high-quality, end-of-life care to patients in their homes, assisted living facilities, and other residential settings. The RN Case Manager is responsible for coordinating and delivering comprehensive hospice care, advocating for patients’ needs, and supporting families during the most meaningful stage of life. This is a field-based role requiring regular travel within the assigned territory.

Key Responsibilities:

Provide skilled nursing assessments and develop individualized plans of care in collaboration with the interdisciplinary team.

Manage a caseload of hospice patients, ensuring high-quality, timely, and patient-centered care.

Perform regular visits to assess physical, emotional, and spiritual needs and adjust care plans as necessary.

Educate patients and families on disease progression, symptom management, and end-of-life expectations.

Coordinate care with physicians, social workers, chaplains, and home health aides.

Administer medications and treatments according to physician orders and hospice protocols.

Document all patient interactions, changes in condition, and interventions in the EMR in a timely manner.

Participate in interdisciplinary team meetings, on-call rotation, and quality assurance activities.
